43 SAMPLE YEARLY INSPECTION CHECKLIST This checklist is intended to document yearly safety checks of outdoor play areas, education exhibits, and other facilities (e.g., decks, stairways, etc.) within the play area. Yearly inspections are done to reduce the potential for accidents and injuries. Check to see if anchors on equipment are secured. Ensure equipment is free of entanglement and protrusion hazards. Ensure equipment is free of sharp points and edges and crush and shear hazards. Check to make sure stepping surfaces are stable and footholds and rungs are tight and free of excess wear. Check that wood equipment is free of rot, splinters, and warping. Check for rusty equipment. Check for cracks in pavement or uneven surfaces that may need repaired or replaced. Check all nearby sidewalks and trails for trip hazards (heaved areas), loose gravel over cement, etc. Check that hand gripping components are secure and do not rotate. Check to see if unitary surfaces are intact and free of depressions and ruts. Check for areas with pooling water or poor drainage, erosion, wasp nests, poisonous or thorny plants, or other natural hazards. Check for broken supports or anchors on equipment and general infrastructure of the play area. Check for lack of lubrication on moving parts. Check loose-fill surfacing is level and at proper depth (i.e., wood chips, pour-in-place rubber, etc.) under and around play equipment (fall zones). Check for chipping or peeling paint, and sharp edges and points on all equipment, buildings, decks, displays, etc. INSPIRATION PLAN DESIGN MANAGE EXAMPLES
